Shoulder Pain Isn’t Always a Shoulder Problem

Shoulder pain can make even the simplest tasks frustrating. Reaching into a cupboard, lifting groceries, putting on a jacket, or brushing your hair can suddenly become difficult and uncomfortable.

When most people experience shoulder pain, they assume there must be something wrong with the shoulder itself. Sometimes that’s true. Conditions such as rotator cuff injuries, tendinopathy, bursitis, or arthritis can all contribute to shoulder pain and restricted movement.

However, not every case of shoulder pain originates from the shoulder.

At Foundation Physiotherapy & Wellness, one of the most important parts of our assessment process is determining what is actually driving a person’s symptoms rather than simply treating where the pain is felt.

Recently, one of our physiotherapists worked with a patient we’ll call Diana.

The Problem Was Gradually Getting Worse

Diana worked as a receptionist and administrative professional. Like many office workers, she spent most of her day sitting at a computer, answering emails, managing schedules, and handling administrative tasks.

Over time, she began noticing increasing stiffness through her neck, upper back, and shoulders. At first, it was nothing more than an annoyance.

Then it started affecting her day-to-day life.

Reaching overhead became uncomfortable. Long days at her desk left her feeling tight and restricted. Her shoulders felt stiff, and she noticed increasing tension throughout her neck and upper back.

When the symptoms continued to worsen, she booked an assessment with our team.

Looking Beyond the Shoulder

One of the biggest mistakes people make when dealing with pain is assuming the painful area is automatically the source of the problem.

During Diana’s assessment, we examined not only her shoulder but also her neck, upper back, shoulder blade mechanics, mobility, strength, movement patterns, and daily demands.

Before developing a treatment plan, we first ruled out more serious causes of shoulder pain, including significant rotator cuff pathology, frozen shoulder, cervical spine referral, and other conditions that can present with similar symptoms.

As we worked through the assessment, a pattern began to emerge.

Diana demonstrated:

  • Reduced mobility through her upper back
  • Forward positioning of her head and neck
  • Rounded shoulders
  • Poor shoulder blade control during movement
  • Weakness in key postural and shoulder stabilizing muscles

Individually, none of these findings explained her symptoms. Together, however, they painted a much clearer picture.

Years of prolonged sitting, repetitive desk work, reduced movement variability, and muscular imbalances had altered the way her neck, upper back, shoulder blades, and shoulders were working together.

As a result, everyday movements became more difficult and less efficient, placing unnecessary stress on surrounding tissues.s a result, the movement in her upper body became impaired, leading to her pain and mobility restrictions.

A Plan Built Around the Root Cause

Rather than focusing exclusively on the painful shoulder, we designed a treatment plan that addressed the underlying factors contributing to Diana’s symptoms.

Treatment began by improving mobility through her thoracic spine, shoulders, and surrounding soft tissues. Hands-on therapy was combined with targeted mobility exercises to help restore normal movement.

As her mobility improved, we shifted our focus toward strength and control.

Diana worked through a progressive exercise program designed to improve:

  • Shoulder blade control
  • Upper back strength
  • Rotator cuff function
  • Postural endurance
  • Overall movement efficiency

We also discussed practical strategies for managing the physical demands of her workday.

Contrary to popular belief, better posture is not simply about “sitting up straight.”

Instead, we encouraged Diana to create more movement throughout her day, vary her positions regularly, and avoid remaining in any one posture for prolonged periods.

Small changes, performed consistently, often have a greater impact than trying to maintain a “perfect” posture.

The Result

Over the following weeks, Diana experienced significant improvements in both pain and function.

Her shoulder mobility improved.

The tension in her neck and upper back decreased.

She was able to work comfortably throughout the day and return to everyday activities without constantly thinking about her shoulder.

Most importantly, she understood why her symptoms had developed and had a clear plan to keep them from returning.

Many people spend months treating shoulder pain by stretching, resting, or avoiding aggravating activities without ever identifying the true source of the problem.

In some cases, the shoulder is the issue.

In others, the shoulder is simply where the symptoms appear.

Brie Asparagus Prosciutto Bundles
🍽️ Brie, Asparagus & Prosciutto Bundles

Ingredients

  • 6 Tbsp unsalted butter, melted
  • 1 Tbsp honey
  • 1/4 tsp kosher salt
  • 1 bunch asparagus, trimmed
  • 1 Tbsp olive oil
  • All-purpose flour
  • 2 sheets puff pastry
  • 12 slices prosciutto
  • 1 (8-oz.) Brie, cut into 12
  • 1 large egg
  • Black pepper

Instructions

  1. Preheat oven to 400°F and line baking sheets.
  2. Mix butter, honey, and salt in a bowl.
  3. Toss asparagus with olive oil.
  4. Roll puff pastry into rectangles and cut into 12 squares.
  5. Brush with butter mixture and layer prosciutto.
  6. Add asparagus and Brie.
  7. Fold pastry, seal with egg wash, and season.
  8. Bake ~15 minutes until golden and melted.
  9. Brush with remaining butter mixture before serving.
