Discovery Call
- After completing your interest form, you will be contacted by our home care lead
- Based on your geographic location and specific needs, our home care lead will pair you with one of our highly trained Foundation @HOME therapists.
- Your physiotherapist will call you, or your loved one, directly to schedule the initial assessment and arrange the first visit.
Initial Assessment
- As with clinic-based physiotherapy, the first part of the process is for the physiotherapist to conduct an initial assessment.
- We will get a complete picture of your symptoms, limitations, needs, and goals.
- At the initial assessment, or soon after, you will be given a personalized treatment plan which outlines the frequency of visits and expected goals.
- The treatment plan will be delivered to you either written or digitally and can be shared with your circle of care.
Follow up Sessions
- For most patients, follow-up sessions will consist of exercises to improve your health condition and help you maintain function and quality of life.
- We’ll work with you to set realistic goals and bring along any special equipment if required.
- Sessions are 45 to 60 minutes long
At the moment, we are only offering in-home physiotherapy. We will expand to in-home chiropractic and massage therapy, as we expand our program. We want to help you regain independence, achieve a greater quality of life, and live better.
