Who is home care physiotherapy right for?
Foundation @HOME is designed for clients whose conditions make it difficult or impossible to travel to a clinic, whether due to limited mobility, post-surgical recovery, a neurological condition such as MS, Parkinson’s, cerebral palsy, or stroke, or a workplace or motor vehicle accident injury. It’s also an excellent option for those managing chronic pain or seniors who would benefit from a more accessible care setting. Our highly trained home care physiotherapists deliver the same standard of clinical excellence you’d receive at any of our seven Foundation locations, the only difference is that we come to you. If you’re unsure whether home care is the right fit, book a free discovery call with our team.
Does home care physiotherapy in Toronto cover the same treatments as a clinic visit?
Yes. Foundation @HOME physiotherapists are registered, highly trained practitioners who carry the full Foundation standard of care with them. Your at-home sessions can include advanced manual therapy, therapeutic exercise prescription, movement investigation, and client education as part of your personalized FUELED treatment plan. The goal remains the same as any Foundation session: identify the root cause of your pain, restore your movement confidence, and set you up to move better, feel better, and live better. Coverage areas include East York, Beaches, Scarborough, Midtown, North York, Downtown, Liberty Village, Little Italy, Ossington Village, and both South and North Etobicoke. Who is Foundation @HOME designed for?
Foundation @HOME brings Foundation’s expert physiotherapy care directly to people who have difficulty or are unable to access one of our clinics.
It can be particularly helpful for people recovering from surgery, experiencing mobility or accessibility limitations, or living with neurological conditions such as multiple sclerosis, Parkinson’s disease or the effects of a stroke.
Instead of allowing travel, accessibility or physical limitations to become barriers to rehabilitation, our highly trained physiotherapists come to you.
How is an in-home assessment different from a clinic visit?
One of the advantages of home physiotherapy is that we can assess and treat you in the environment where you actually live and move every day.
That gives your physiotherapist the opportunity to understand your real-world needs, incorporate your home setup directly into your exercise plan and identify practical barriers that may be affecting your mobility, safety or independence.
Home care can also make it easier to involve family members or other health professionals in your rehabilitation when appropriate.
It’s still Foundation physiotherapy — just built around your environment, your needs and your goals.
What can I expect during my first Foundation @HOME visit?
Just like an in-clinic physiotherapy appointment, your Foundation @HOME journey begins with an initial assessment.
Your physiotherapist will develop a complete picture of your symptoms, limitations, needs and goals. From there, we’ll build an individualized treatment plan designed to help you regain movement, strength, fitness, function and independence.
Before your first visit, our home care team will also help coordinate your care. After completing an interest form, our home care lead will connect with you, determine your needs and geographic location, and pair you with an appropriate Foundation @HOME physiotherapist.
It’s our commitment to clinical excellence and outcome-based physiotherapy — brought directly to you.
